How to apply
Apply by filling in the form by clicking on the button below. A three-month, six-month or nine-month permit may be extended to a maximum of 12 months from the date of issue. Please ensure your medical practitioner has filled out the required sections.

How to extend
To extend a short term permit, please visit your medical practitioner and ensure they have completed the medical page of the above application form. You will need to fill out the rest of the form, including the method of payment, and return it to us following the instructions on the application form.

Short-term permit: $35.00
Extension: $10.00
