Hours: Permanent, full-time (35 hours per week)

The role as a Service Coordinator is responsible to work in partnership with the individuals and their families to develop, deliver and/or coordinate a range of person-centred services that aim to empower and enable the individual to achieve their personal lifestyle goals.
This role also provides advocacy services for people and their whanau within a network of agencies and state providers, such as Ministry of Education, Local Schools, Health NZ, Whaikaha, WINZ, Kainga Ora and Oranga Tamariki to name a few.
With the support of Nelson Marlborough Team, you’ll:
Coordinate and deliver high quality services that are creative, flexible, and responsive to the needs of disabled people and their families/whānau. Services focus on improving/maintaining inclusion of disabled people within their families/whānau and the wider community.
Maintain a high standard of professional and organisational behaviour and practice.
Recognise safety issues for children, young people and vulnerable adults, respond to these and refer to appropriate supports/organisations (internal and external).
Develop and maintain community connections and networks; and promote the concept of inclusion and community development.
Develop and maintain positive working relationships with disability consumer groups and governance.

About you
We are looking for people who:
Have relevant or equivalent qualification in human or social service and/or work experience.
Have an understanding of working with Families, Youth and Adult and the ways to support people to continue to grow and develop and to work across a variety of contracts.
Have strong commitment to providing quality support with positive action and outcomes to disabled people and their families.
Are a person that looks for opportunities to develop and maintain community connections and networks with a focus on supporting the community towards greater inclusion for all people.
Have understanding of Te Tiriti o Waitangi.
Have an understanding and knolwedge and/or experience working with disabled people would be a distinct advantage.
Have a full/restricted drivers licence or access to transport.
Have an ability to form positive relationships and networks.
Have great oral and written communication skills.
Have strong time management, organisational and reporting skills.
