Job description Community Support Worker

Job location: Napier, Hastings, Flaxmere

Hours: Varied

Flaxmere – Intensive Wraparound Support (Rangatahi & Whānau):

Up to 12 hours per week (after school hours). This role involves providing intensive, wraparound support to a rangatahi and their whānau. You will work alongside them to build strengths, respond to complex needs, and support the achievement of their goals and aspirations in a holistic, whānau-centred way.

Hastings – Supported Independent Living (Adults):

Up to 15 hours per week (business hours). Supporting adults to live as independently as possible within their community. This includes assisting with daily living skills, goal setting, and promoting autonomy, choice, and participation.

Napier – Supported Independent Living (Adults):

Up to 10 hours per week (business hours). Providing community-based support to adults, focusing on building independence, enhancing wellbeing, and supporting individuals to achieve their personal outcomes.

Job specific information

As a Support Worker, you’ll:

  • Ensure support is provided in ways, at the times and places agreed in the individual’s Personal Plan with CCS Disability Action.

  • Provide services in accordance with CCS Disability Action policies and procedures and work practices and the Disability Leadership Framework.

  • Be respectful of the individual and their home.

  • Remain professional at all times and in line with CCS Disability Action’s Standard of Integrity and Conduct.

  • Take part in supervision, performance review and training opportunities.

  • Attend compulsory training sessions that CCS Disability Action offer.

About you

We’re looking for someone who:

  • Can ensure disabled people have the ability to reach their identified goals in an enabling, culturally respectful and unobtrusive manner.

  • Can ensure that the support work is driven by the person we support.

  • Can be creative, flexible, and responsive in your approach.

  • Can provide support which encourages and facilitates the individual’s inclusion in their family/whānau and wider community.

  • Has a clean driver’s license or access to transport.

  • Has up to and including Level 2 training or be willing to achieve Level 2 training.
