Job description Community Support Coordinator
Hours: Permanent, part-time (32 hours per week)
The Community Support Coordinator, a member of the Northland Team of CCS Disability Action Northland Branch, has the responsibility to work in partnership with individuals and their families to develop and implement a range of flexible community options which enable them to remain strong and connected to their family and community. All services are developed and delivered in such a way that they reflect our commitment to supported lifestyles and community development and are in line with the organisations’ strategic priorities document.
Job specific information
We are looking for an enthusiastic and resourceful applicant to join our diverse team in the position of a Community Support Coordinator. This role covers a range of activities including coordinating individual services to disabled people and their families and/or working on community activities which will remove barriers to participation. The holder of this position will work in partnership with disabled people and their families throughout Northland to develop, deliver and/or coordinate a range of person directed services that aim to empower and enable the individual to achieve their personal lifestyle goals. The successful person will have some responsibility for leading and supporting support workers, to ensure disabled people receive the support they want and need. Ideally this person will be based in, or near, Whangārei.
As a Community Support Coordinator, you will:
Have good community networks.
Have experience in the social sector.
Have understanding of the disability sector and ability to relate to disabled people in a positive affirming manner.
Have an understanding of and are committed to the rights of disabled people.
With the support of our organisation, you’ll:
Work as part of a team of Community Support Coordinators, support workers and Northland Service Manager.
Work in partnership with whānau/families to develop and implement a range of flexible community options which enable them to remain strong and connected to their family and community.
Our organisation takes a social, rather than medical approach, supporting disabled people to live their best lives. We are guided by the Enabling Good Lives principles, shifting power and authority back to disabled people and their families, to increase choice and control in their lives.
About you
We are looking for people who:
Have 2-3 years minimum experience of working together effectively with disabled people and their families to achieve agreed outcomes – in a coordination related capacity. This role is not for someone looking to be a support worker.
Have excellent skills in developing and maintaining trusting and accountable relationships with staff.
