What is transition?The transition service supports young people to move from education to the next stage of their lives.
Why is Transition important?This is a key period for young people and must begin during their secondary school years. It requires a holistic view of each person’s life. Getting a job should not be the primary focus of whatever programme of support (known as the Individual Living Plan or ILP) is decided on between Supported Lifestyles, the young person and their whanau and family. The principle objective must be the successful negotiation of each person’s teenage and early adult years.
The Independent Living PlanILP outcomes therefore should be directed towards defining skill development and social and recreational opportunities in a wide range of areas. This is necessary to:
· Ensure that learning objectives set within the ILP will enable the young person to become a self-determining, competent young adult
· Establish the range of community experiences that will enable the young person to experience social and recreational opportunities in their local community
· Establish a career pathway with the young person and their school at least two years before school-leaving
· Assist the young person find a job or undertake further training or community involvement before leaving school.
What does the service do?The transition service can help disabled young people aged 16-21 who are getting ready to leave school or tertiary education to:
· Try out some work experience placements
· Look for after school or holiday work
· Find out about study options
· Think about how you will spend your leisure time
· Link in with other services
· Explore community recreation and leisure options
Who can use these services?We work with young people who:
· Are OORS funded and in their last year of school
· Have VHNORRS transition funding and want to explore community focused work and leisure options · We may also work with young people who are not ORRS funded.
· We can also give advice to schools that are supporting young people with disabilities.
· We accept referrals from young people, families/whanau and schools.
